Public Water Distribution Systems in India - Where do we stand for identifying the causes for leakage?

Several efforts have been taken to arrest leaks, quantify leaks, replace valves and pipes, and restore water supply. But there is no effort taken to study the cause of leakage. Below clippings show the seriousness of such problems and alarm authorities to widen their focus on identifying the causes for failures and finding proper control measures.
